A request for facts the bank has recorded
This format is for asking the bank to identify an account restriction and the information it needs from you. It is useful when the explanation you have is incomplete: a failed transaction or a short branch conversation may not give you the instruction reference, affected amount or issuing authority.
In this example, Aarav recorded an inaccessible account amount of ₹86,500. The branch reported a cyber complaint connected with a ₹4,000 transaction. Those are separate facts. The complaint reference and authority contact details were still unknown; the letter asks for them instead of supplying invented identifiers.
The sample requests information. It does not establish that the restriction is unlawful, that the bank must disclose every requested record, or that Aarav is entitled to immediate account release.

If you have no recorded amount, reference or date, do not enter zero, an approximate identifier or a date chosen to complete the template. Say what is unknown and ask for clarification. The worked amounts example shows why transaction, disputed and inaccessible amounts need their own labels.
What the example asks the bank for
The questions cover the recorded reason and date, the instruction or correspondence reference, a relevant copy where applicable, and any recorded issuing-authority contact. They also ask whether other restrictions exist, what information or documents the bank needs, and how the request will be acknowledged.
Keep a copy of what you send. Record the actual date, recipient, channel and any acknowledgement rather than assuming that preparing a letter means it was delivered. You can then use the follow-up example to refer to that specific communication and the reply you actually received.
